Director of Market Growth & Strategic Partnerships

Organization: Home Centered Care Institute (HCCI)

    Reports to: CEO

    About HCCI

The Home Centered Care Institute (HCCI) is a collaborative non-profit with a mission of increasing access to home-based primary care (HBPC), thereby improving the quality of life for medically-complex patients and their families. HCCI is passionately committed to its vision of bringing HBPC into the healthcare mainstream. In addition to being an authoritative source for clinical and operational best practices, HCCI is advancing the field of HBPC through education, consulting, research, and advocacy.

HCCI is a unique nonprofit, operating at the intersection of a consulting firm, an educational leader, and a data analytics lab. Our urgent mission is to serve the nearly 6 million people in the U.S. who are homebound, home-limited, or living with serious illness and do not yet have access to best practice house call programs. Our organizational culture values creativity, empathy, and the relentless pursuit of a better healthcare system.

    Role Overview

The Director of Market Growth & Strategic Partnerships will architect the expansion of a movement.

This person will identify and nurture long-term alliances with health systems, ACOs, payers, and private practices, providing them with the intellectual capital (education) and business intelligence (data analytics) they need to succeed in the future of healthcare. This is a leadership position for someone who enjoys building something durable - relationships and revenue - while advancing a mission that matters.

The position carries clear revenue accountability and ambitious growth goals, while requiring a consultative, mission-aligned approach that reflects HCCI’s nonprofit values and standing in the home-based care community. This position does not manage a team, but rather, drives and owns the process themself, in collaboration with other HCCI team members.

HCCI seeks a growth-oriented professional who understands that lasting revenue is built through trust, credibility, and long-term partnerships and not just transactional selling alone. The Director will develop and steward relationships with provider organizations, population health leaders, and healthcare partners, translating HCCI’s education, consulting, and data analytics offerings into sustained value for the field.

    Primary Responsibilities

    Market Development & Revenue Growth

    • Develop and execute a multi-year earned revenue strategy that supports significant organizational growth and sustainability.
    • Build and manage a diversified portfolio of organizational relationships across provider practices, health systems, ACOs, payers, and aligned partners.
    • Identify, cultivate, and advance new opportunities by deeply understanding prospective partners’ clinical, operational, and value-based care priorities.
    • Guide organizations through a thoughtful engagement process - from initial conversation through contracting - positioning HCCI as a trusted advisor and long-term partner.

    Relationship-Centered Sales Execution

    • Conduct proactive outreach and follow-up that balances new opportunity development with stewardship of existing relationships.
    • Deliver compelling, mission-anchored presentations and proposals that clearly articulate impact, value, and return on investment.
    • Negotiate and finalize agreements in a manner that supports mutual success and long-term collaboration.
    • Maintain a disciplined sales pipeline and forecasting process using CRM tools, with an emphasis on quality of relationships as well as velocity.

    Collaboration & Field Presence

    • Partner closely with Education, Consulting, Research, Marketing, and Development colleagues to ensure alignment between offerings, messaging, and market needs.
    • Represent HCCI at conferences, convenings, and industry events to deepen relationships, expand visibility, and generate strategic opportunities.
    • Serve as a connector across the field, identifying trends, unmet needs, and partnership possibilities that inform program development and organizational strategy.

    Performance & Learning

    • Meet clearly defined revenue and growth targets, with success measured across both short-term results and long-term portfolio strength.
    • Track and report on agreed-upon metrics related to revenue, engagement, retention, and relationship development.
    • Continuously refine approaches based on market feedback, performance data, and evolving organizational priorities.

    Success Indicators

    • Growth in earned revenue across education, consulting, memberships, and data analytics
    • Strength, diversity, and sustainability of relationship portfolio
    • Client retention, repeat engagement, and expansion over time
    • Contribution to HCCI’s reputation, reach, and influence within home-based care
    • Effective collaboration with internal teams and external partners
    • Fitting in to the HCCI organization culture

    Qualifications and Experience

    • 5+ years of experience in business development, sales, or fundraising, preferably in healthcare, consulting, education, or nonprofit earned-revenue environments.
    • Demonstrated success building and sustaining relationships that lead to meaningful, long-term revenue growth.
    • Comfort operating in both strategic and hands-on modes, from high-level relationship cultivation to detailed opportunity management.
    • Strong communication, presentation, and negotiation skills, with the ability to educate and influence senior leaders.
    • Experience using CRM systems (preferably HubSpot) and data to guide decision-making and performance management.
    • Alignment with HCCI’s mission and values; appreciation for the role of nonprofit organizations in advancing systemic change.
    • Bachelor’s degree, preferably in business or marketing
    • Willingness to travel (~25%) and participate in occasional evening or weekend events.

    Personal Attributes

    • Confident and Informed Spirit: Someone who understands how to "make the ask" in the context of a larger mission.
    • Healthcare Fluency: A working knowledge of the home-based care landscape, value-based care models, or healthcare analytics is highly preferred.
    • Entrepreneurial Agility: Comfort building your own “playbook” and pivoting when you find a new way to create value for our partners.
    • High EQ: Emotional intelligence to navigate complex organizational structures and build consensus among diverse stakeholder groups (from physicians to CFOs).
    • Skillful Connector: Playing the long game and being able to build trust with population health leaders and executives by understanding their pain points and offering HCCI as a strategic solution.
    • Advocate with a Business Mind: Able to speak fluently about value-based care, ROI, and data analytics, and be driven by the story of the homebound patient who finally gets the care they deserve.
    • Market Sculptor: Identify gaps in the healthcare landscape and position HCCI’s programs to fill them.

    Core Compensation Structure

    • Base Salary: $125,000
    • Maximum Incentive: 30% of base salary ($37,500)
    • Total Potential Compensation: $162,500

    Performance Metrics & Weighting

An annual incentive pool is earned based on the "totality of work" across three weighted pillars:

    1. Sustainability (35%): Total earned revenue from all sources (Consulting, Education, Data), aligns to strategic priorities, emphasizes multi-year and repeat partnerships, and says no to misaligned opportunities.

    2. Stewardship (25%): Number of new partnerships signed (Health Systems, ACOs, Payers), depth of engagement, conversion of prospects to long-term partners.

    3. Quality (20%): On-time delivery of contract milestones and execution on client commitments.

    4. Outcomes (20%): Documented impact (from partner data, testimonials, and case studies), positive feedback from program teams and leads, accurate handoffs, supporting mission outcomes beyond revenue, and helping others succeed.
